Project Manager (ASWO)

Job Description

  • Manage and coordinate different new service projects of the Association

  • Serve as case manager of people with dementia and their families, e.g. care planning, counseling and caregivers support, etc.

  • Participate in new developments of the Association to fill in service gaps of dementia care in Hong Kong

  • Represent the Association to provide training and talks to other organizations

Requirements

  • Registered Social Worker with Bachelor Degree or above

  • With working experience in elderly and/or dementia care is preferred

Occupational Therapist I

Job Description

  • Implement early detection assessments and provide clinical judgement to facilitate diagnostic process for people suspected with dementia in the community.

  • Implement outreach in-home service to families living with dementia, e.g. cognitive and self-care training, home modification and providing caring advices etc.

Requirements

  • Registered Occupational Therapist with degree qualification, at least 5-year working experience in elderly and/or dementia service.

​

Salary Range

  • HK$54,805 to HK$79,930

  • 文憑或以上程度，熟悉電腦操作，懂Adobe Illustrator/ Canva/影片製作者優先

註冊社工 (SWA) - 日間中心 (九龍 / 港島區)

​

<工作範圍>

  • 支援服務經理處理日間中心運作，個案及照顧者跟進

  • 於認知障礙症日間中心推展治療小組及家屬支援工作

​

<入職要求>

  • 具至少兩年相關經驗

活動工作員Program Worker - 日間中心 (九龍區)

​

<工作範圍>

  • 籌劃中心大型活動及帶領認知刺激及社交小組

  • 支援日常患者照顧及中心運作

​

<入職要求>

  • 具至少兩年相關經驗

照顧員 (PCW) (九龍 / 新界區)

​

<工作範圍>

  • 協助認知障礙症日間中心運作; 服務使用者個人護理及接送；中心及辦公室清潔

  • 星期六長短週工作，不用輪班工作，逢公眾假期休息。

  • 通過試用期後每月可獲 $1000現金津貼

​

<入職要求>

  • 具長者服務經驗優先
